Can I still receive dental care, even if I am not a DHC medical patient?

Yes. You do not need to be a medical patient to receive dental care.

What if I need to see a specialist?

DHC providers refer patients to specialists.  After receiving the referral, it is your responsibility to make future contact with the Specialist along with payment for their services.

What if I can’t afford my prescriptions?

If you cannot afford your medications, please ask our staff about different assistance programs that may be available.  We offer a Prescription Assistance Program for those without prescription coverage who meet the income guidelines.

How can I obtain a copy of my medical records?

To obtain medical records, a Release of Information form must be completed by the patient.  Fees may be assessed depending on the number of pages requested.

What if I can’t make my scheduled appointment?

Please notify the clinic at least 24 hours prior to the appointment.

What is a Federally Qualified Health Center?

A federally qualified health center is a local, non-profit health care center that provides comprehensive, primary, affordable care to all residents regardless of an individual’s income or insurance coverage.  These community health centers are located in areas where care is needed but in short supply.
